Social Prescribing Link Worker x 2
Chorley Council are looking to recruit 2 x Social Prescribing Link Workers, on a fixed term basis until March 2022
1 x Full Time Post (36.25 hrs per week)
1 x part Time Post (33 hours per week)
Following successful delivery of the first 12 months of the Social Prescribing Service, the council is working with local GPs to further increase capacity of the existing social prescribing service. Social prescribing recognises that people’s health is determined by a range of social, environmental and economic factors. It aims to support a person to improve their wellbeing and resilience by providing them with opportunities to achieve their goals.
You’ll need to enjoy working with people, with skills in health coaching techniques and a good knowledge of public services. You will be able to manage a caseload effectively, from initial assessment through to case closure and evaluation.
