Patients are invited to join our Patient Participation Group
The Chorley Surgery was formed from the merger of Dr Bamford and Partners and Cunliffe Medical Centre and we are focused on developing the best practice for you, our patients. Your involvement in the development of the Practice is essential if we are to remain true to our mission to be “striving for excellence in healthcare for you and your community”.
We are inviting patients to join our Patient Participation Group. The group will be asked to complete a series of surveys on Practice services during a year ( not too many!!) and will also be asked for opinions on changes proposed in the way the Practice works from time to time.

All information we ask for is fully protected under NHS Information Governance and Confidentiality rules and is following the Data Protection Act 1998 (as amended).
You must be a registered patient of The Chorley Surgery to take part. We want to represent all of our patients and therefore need a broad cross-section of our patients in the group- young people, senior citizens, different ethnic groups, men and women, and different medical conditions.
