Every patient is assessed to ensure they get access to the healthcare they need. If you have a long-term chronic condition, we might ask to see you at least annually, to check how things are going and to see whether we can improve the management of your condition.
If we’ve invited you for an annual review, please make an appointment if you haven’t already; we’d love to talk to you about how things are going.
