Clinical Pharmacist x 2

We are seeking to recruit two PCN Clinical Pharmacists to join our growing pharmacy team.

The post holders will help to develop and support medicines optimisation work within our 6 GP practices in the PCN. They will work as part of a multi- disciplinary team and have a patient-facing role. The post holders will take responsibility for areas of chronic disease management and work proactively to manage people with complex polypharmacy and those with multiple co-morbidities with other healthcare professionals as part of a multidisciplinary team across the PCN, as well as conducting regular medication reviews.

Part time Chorley Central PCN Manager

Chorley Central PCN are excited to be recruiting a PCN Manager.

The successful candidate will work alongside the PCN Clinical Director, Board Members, other local PCN Managers to plan, implement and manage all operational aspects of our PCN and to deliver services in line with current guidelines and the PCN DES.

To provide line management to all PCN employed staff to ensure they are supported and the PCN maximises staff retention and skills development.

PCN Paramedic Practitioners x 2

Chorley Central PCN comprises of 6 GP surgeries and covers a population of over 54,000 patients and are expanding our team to meet the needs of our population. Our Model of Care will see you working alongside a multi-disciplinary team of experienced GPs, Nurse Practitioners, Clinical Pharmacists

Our aim is to provide a team that supports and leads on the proactive care of those patients that are high risk of admission, such as decompensating frailty, End of Life and/or long term conditions, by providing routine checks and urgent visits.
