Chorley Council is working with local GPs to deliver the successful social prescribing service and support a population health management approach.
Social prescribing recognises that people’s health is determined by a range of social, environmental and economic factors. It aims to support a person to improve their wellbeing and resilience by providing them with opportunities to achieve their goals.
You’ll need to enjoy working with people, with skills in health coaching and motivational techniques and a good knowledge of public services. You will be able to manage a caseload effectively, from initial assessment through to case closure and evaluation.
